Mindfulness Questionnaires

I’ve been involved in the development of several questionnaires designed to measure mindfulness or related variables.

Kentucky Inventory of Mindfulness Skills

This instrument is designed to measure four mindfulness skills: observing, describing, acting with awareness, and accepting without judgment. It was inspired by the mindfulness skills described by Marsha Linehan in dialectical behavior therapy (DBT) and by other contemporary descriptions of mindfulness. More information is available in:

Five Facet Mindfulness Questionnaire (FFMQ)

This instrument is based on a factor analytic study of five independently developed mindfulness questionnaires. The analysis yielded five factors that appear to represent elements of mindfulness as conceptualized in the psychological literature. The five facets are observing, describing, acting with awareness, non-judging of inner experience, and non-reactivity to inner experience. More information is available in:

Acceptance and Action Questionnaire – II (AAQ-II)

This instrument is designed to assess psychological inflexibility or experiential avoidance, as defined by Steven Hayes and colleagues in acceptance and commitment therapy (ACT). It has seven items and a single factor structure. More information is available in:

Avoidance and Fusion Questionnaire for Youth (AFQ-Y)

This instrument is designed to measure cognitive fusion and experiential avoidance, two important elements of psychological inflexibility as defined in ACT. It has a single factor structure and is available in two versions: the standard 17-item version and an 8-item short form. Although designed for youth over the age of 9 years, evidence suggests it may also be useful in adults. More information is available in:

Child and Adolescent Mindfulness Measure (CAMM)

This instrument was designed to assess mindfulness skills in youth over the age of 9 years. It was based loosely on the KIMS and includes 10 items assessing lack of awareness of ongoing activity and judgmental or avoidant responses to thoughts and feelings. It has a single factor structure.
